چکیده

Case presentation: A 76-year-old man with rate-controlled atrial fibrillation (AF), diabetes mellitus, and prior stroke who is receiving warfarin to prevent recurrent stroke presents to the emergency department with chest pain, elevated serum troponin, and an ECG that demonstrates ST depression in the precordial leads. Cardiac catheterization reveals an ulcerated plaque and partially obstructive thrombus in the left circumflex coronary artery. Percutaneous coronary intervention is performed with placement of 2 baremetal stents. What is the optimal antithrombotic therapy? What is the optimal antithrombotic therapy if the patient receives drug-eluting stents instead of bare-metal stents?

Triple antithrombotic therapy in patients with atrial fibrillation undergoing coronary artery stenting: hovering among bleeding risk, thromboembolic events, and stent thrombosis

Dual antiplatelet treatment with aspirin and clopidogrel is the antithrombotic treatment recommended after an acute coronary syndrome and/or coronary artery stenting. The evidence for optimal antiplatelet therapy for patients, in whom long-term treatment oral anticoagulation is mandatory, is however scarce. To evaluate the safety and efficacy of the various antithrombotic strategies adopted in ...

Antithrombotic therapy in patients treated with oral anticoagulation undergoing coronary artery stenting. An expert consensus document with focus on atrial fibrillation.

Dual antiplatelet treatment with aspirin and clopidogrel is recommended after coronary stenting (PCI-S). There is scant evidence defining optimal post-PCI-S antithrombotic therapy in patients with atrial fibrillation (AF) in whom oral anticoagulation (OAC) is mandated. To evaluate the safety and efficacy of the antithrombotic strategies for this population, we conducted a systematic review of t...

Triple therapy for atrial fibrillation and percutaneous coronary intervention.

PURPOSE OF REVIEW Patients with atrial fibrillation undergoing percutaneous coronary intervention (PCI) require treatment with oral anticoagulation (OAC) and additional dual antiplatelet therapy with aspirin and clopidogrel (DAPT), i.e. triple therapy. However, triple therapy produces a high annual bleeding risk outweighing the benefits. To improve safety of antithrombotic treatment in these pa...

coagulant and single antiplatelet therapy (SAPT)) without enhanced risks of vascular events.7,8 In the ISAR-TRIPLE Trial,9 which compared TT of different durations (6 months and 6 weeks), in AF patients undergoing interventions with drug-eluting stents (DES), there were no significant differences in cardiac death, myocardial infarction, definite stent thrombosis, and ischemic stroke. At an indi...
